• Weight Management Service (virtual face-to-face required)

💻 How It Works

1. Book an Appointment:
       Schedule online via www.Healthify.Clinic
2. Choose Your Consultation Type:
   • Video Consultation: via a secure platform (Writeupp)
   • Telephone Consultation via secure platform
   • Message a Doctor via secure platform

1. Attend Your Appointment:
   • Ensure you are in a private and quiet space.
   • Use a strong internet connection for video calls.
   • Be ready to discuss your symptoms, medical history, and concerns.

1. After the Consultation:
   • You may receive prescriptions, a follow-up plan, fit notes or specialist       referrals letters.
   • Notes and letters will be emailed securely to your Email ID and/or       to your NHS GP with your Consent.

     

💊 Prescriptions

• E-prescriptions will be sent directly to your phone or email. However, on some occasionals, we can send      them to your local pharmacy with your consent.
• Controlled medication may require additional verification.
• Bridging Private Scripts for certain Repeat prescriptions are      available upon clinical review.

📁 Confidentiality & Data Protection

We are fully compliant with the UK GDPR and Data Protection Act 2018. Your personal and medical information is handled with strict confidentiality and stored securely.

Please call 999 or go to A&E if you experience:

• Chest pain or shortness of breath
• Symptoms of a stroke
